Intake forms: collecting what you need before the appointment

Some services need information before you can safely or properly deliver them - a patch test history, a medical condition, a style reference. Intake forms collect that as part of booking, instead of you asking at the door.

What intake forms are for

Anything you’d otherwise ask on a paper form or over text before an appointment: allergies, previous treatments, consent for a specific procedure, or just a preference you’d rather know in advance than guess on the day.

Setting one up

From Services → [service] → Intake form, add the questions you need - short text, multiple choice, or a yes/no consent checkbox. Attach it to any service where it’s relevant; not every service needs one.

Per-staff intake forms

On Starter and Pro with the Team add-on, a staff member can have their own intake form independent of the service itself - useful if two staff doing the same service ask different things.

Where responses show up

A completed intake form attaches to that booking and to the client’s profile, so the answers are there in your calendar the moment the appointment starts - no separate form to go looking for.
